Why PA Role Is Growing in India
India has 1 doctor for every 834 people (WHO 2024). For specialists, the ratio is dramatically worse — particularly in cardiology, nephrology, oncology, and transplant medicine. Hospitals can't manufacture more specialists. So they hire PAs to extend each consultant's reach.
Apollo Hospitals — India's largest PA employer with 1,200+ PAs across its network
Fortis, Medanta, Manipal, KMCH, MIOT, Christian Medical College Vellore all hire PAs in cardiology, nephrology, oncology, and surgical units
Transplant centres (kidney, liver, heart) treat PAs as indispensable — managing pre-transplant workup, post-op care, and immunosuppressant monitoring
IVF and fertility clinics increasingly hire PAs for patient counselling and procedure assistance
The Indian Association of Physician Assistants (IAPA) is also lobbying for formal regulatory recognition — which would significantly elevate salaries and career security in the next 3–5 years.
Day in the Life of a Physician Assistant
A cardiology PA at a corporate hospital starts at 8 AM with ward rounds alongside the senior consultant — reviewing overnight events, presenting cases, documenting plans. From 10 AM to 2 PM you handle OPD assistance: taking patient histories, performing initial examinations, reviewing test reports, drafting prescriptions for the consultant's signature, and counselling patients on lifestyle and medication adherence.
Afternoons might involve cath lab assistance, hospital ward management for admitted patients, coordinating with cardiothoracic surgery for pre-op clearances, and managing chronic patient follow-ups via telemedicine. Senior PAs run dedicated heart failure or hypertension clinics under consultant supervision.
In transplant medicine, PA work is even more demanding and rewarding — managing pre-transplant evaluation, organising donor workup, post-op recovery coordination, and long-term immunosuppression monitoring. PAs become trusted right hands to consultants over years.
Career Paths & Specializations
Cardiology PA — Most common specialization. Works with interventional/non-invasive cardiologists. Strong long-term mentorship
Nephrology PA — Pre-dialysis CKD management, transplant coordination. Niche and well paid
Oncology PA — Chemo protocol management, patient counselling, coordinating multi-modal care. Emotionally demanding but high-impact
Transplant Coordinator PA — Liver, kidney, heart transplant centres. Specialist role with very limited supply
Critical Care PA — ICU support, often pairs with intensivists to manage daily round logistics
Surgical PA — Assists surgeons pre/intra/post-op, particularly in cardiothoracic, neuro, and orthopaedic specialties
Medical Officer (corporate health) — Move into corporate health, insurance, or pharma medical affairs after 4–6 years
